A lesson plan for use in a blended learning classroom that includes the following: title; identification and brief description of the lesson’s primary topic or focus, identification and brief description of the larger unit of study; identification and brief description of the course/subject and grade-level; learning goals and outcomes; learning and/or performance objectives; identification of any related state, common core, or other applicable standards.

The lesson plan also includes: a description of content; description of activities; any related documents or materials (handouts, quizzes, etc.); identification and description of the blended model(s) used for this lesson; description of the face-to-face and virtual environments, with identification of the specific activities to be completed in each environment.

The lesson plan includes: a description of 2+ accommodations (for each category) designed to meet the needs of learners who may benefit from: remedial assistance; customized materials; alternative options for demonstrating mastery; additional time or practice; extension materials; assistance gaining/maintaining interest and/or focus on subject matter; materials/resources that align with the specific culture or faith beliefs for students who represent a minority population within a classroom.
