Care Management: Clinical Issues: 1. Become familiar with the broad scope of clinical geriatric care management. 2. Appreciate the principles of strength-based assessment for later application in care planning. 3. Discern the appropriate application of screening procedures and instruments for psychosocial assessment. 4. Understand Coleman's 4 Pillars of Care Transitions as applied to a complex patient care plan with particular attention to the family caregiver.
Caregiving & Aging Families: 1. Summarize the biological and psychological changes in aging. 2. Discover complex realities about caregiving in the United States. 3. Define the ways caregiving will change in the future and ways you can assist caregivers in decision-making.
